Bus Forms:

For the safety and well-being of all children, each student is required by the State of Illinois School Code to submit documented evidence of a physical and proof of immunizations. The school is required by the State Board of Education to use a standard form issued by the state to record and verify the physical examination and immunization data. This required form must be returned to the school office on or before the first day of class.

Incoming Preschool Students:

Physical Form with immunizations signed by physician and Health History signed by parent/guardian.

Incoming Kindergarten Students:

Physical Form with immunizations signed by physician and Health History signed by parent/guardian.
